摘要:
目的 观察睡眠剥夺对高职业压力护士氧化应激和Th17/IL-17炎症轴的影响。方法 根据睡眠剥夺时间和工作负荷不同将我市某三甲医院急诊科、产房、心内科、脑外科50名女性护士作为观察组,普通病房、门诊和供应室50名女性护士作为对照组。观察组护士实施传统的“8 h三班倒”护理轮班制,每位护士每周上1个前夜班、1个后夜班及正常的日班,每周循环1次;对照组护士工作时间从上午7:30—12:00,下午2:00—5:30,每名护士每天的平均工作时间为8 h。采用匹兹堡睡眠质量指数量表(PSQI)分别测评2组护士的睡眠质量;每天上午7:30抽取2组护士的静脉血,连续1周,采用黄嘌呤氧化酶法检测血清超氧化物歧化酶(SOD)水平,采用TBA法测定血清丙二醛(MDA)含量,采用ELISA法测定血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-17、IL-21含量。结果 观察组睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物、日间功能障碍评分及PSQI总分均高于对照组,2组比较差异均有统计学意义(P<0.01);观察组SOD水平明显降低,MDA、TNF-α、IL-17、IL-21含量明显升高,与对照组比较差异均有统计学意义(P<0.01)。结论 睡眠剥夺可导致高职业压力科室护士氧化应激,并影响炎症因子含量,从而导致人体免疫功能下降。
Abstract:
Objective To observe the effects of sleep deprivation on oxidative stress and Th17/IL-17 inflammation axis of the nurses in the high occupational stress departments. Methods According to the differences of sleep deprivation time and workload in a hospital of Wuhu, fifty female nurses of emergency department, delivery room, department of cardiology and department of cerebral surgery were chosen as observation group, other fifty female nurses of general ward, outpatient department and supply room were chosen as control group. The nurses in the observation group implemented the traditional "8 h three shifts" nursing shift system. Each nurse has 1 first half of the night shift, 1 latter half of the night shift and normal day shifts, cycles once a week. The control group worked from 7:30 to 12:00 a.m. and from 2:00 to 5:30 p.m., and each nurse worked an average of 8 hours a day. We used pittsburgh sleep quality index (PSQI) to evaluate the sleep quality of the nurses in the 2 groups. The venous blood of two groups of nurses was drawn at 7:30 a.m. every day for one week. The serum level of superoxide dismutase (SOD) was detected by xanthine oxidase method, the serum level of malonaldehyde (MDA) was detected by TBA method, and the serum content of TNF-α, IL-17 and IL-21 was determined by ELISA. Results The scores of sleep quality, sleep latency, hours of sleep, sleep efficiency, sleep disorder, hypnotics, daytime dysfunction and PSQI total score of the observation group were higher than those of the control group, there were statistically significant diffe-rences between the two groups (P<0.01);in the observation group, the SOD level significantly decreased, while the content of MDA, TNF-α, IL-17, IL-21 significantly increased, there was statistically significant differences between the two groups (P<0.01). Conclusion Sleep deprivation can result in the oxidative stress of the nurses in the high occupational stress departments, and affects levels of inflammation, thus leads to a decline in the immunologic function of the human body.
